You should complete an incident report if any of the following occur during your game:
Ejections – Players, coaches, or spectators removed from the game.
Unsportsmanlike Conduct – Verbal abuse, physical altercations, or excessive arguing.
Rule Disputes – Significant rule challenges that impact the game.
Injuries – Any injury requiring medical attention.
Game Interruptions – Field issues or external interference.
When completing the report, be sure to:
Be clear and concise – Stick to the facts without opinions or emotions.
Include who, what, when, where, and why – Provide specific names, times, and actions.
Use objective language – Avoid personal interpretations; describe what you saw.
Submit the report within 24 hours – Timely reports help with accurate follow-ups.
Once completed, your report will be reviewed by the appropriate league or umpire association officials. If further action is needed, they will follow up with you for clarification.
